Salinan penyata buku ASB/ASN/ASD/ASW atau Tabung Haji yang mempunyai muka hadapan (maklumat pelabur) dan 3 mukasurat terakhir bertransaksi yang disahkan oleh pegawai bank/institusi berkaitan (nama & nombor staf) berserta tarikh dan cop bank/institusi berkaitan; atau/dan
Surat sokongan pembiayaan kewangan (jika ada).
3. Salinan Kad Pengenalan / Surat Pengesahan Status Bumiputera yang jelas.
4. Salinan akaun Central Depository System (CDS) atas nama sendiri.
5. Bagi pegawai Kerajaan/Badan Berkanun, sila dapatkan surat kebenaran daripada Ketua Jabatan untuk melanggan saham khas ini.

Application for an Initial Public Offering (IPO)
There are two methods which you can apply for an IPO: Panduan Pelanggan Maybank
Through application forms at issuing houses, stockbroking companies and financial institutions.
And sometimes through an Electronic Share Application (ESA) that allows applications via ATMs.
Step 1
Check the newspaper & Bursa Malaysia website for information on Initial Public Offering (IPOs).
Step 2
Before applying for IPOs, carefully read the prospectus.

You can apply for IPOs using application forms.
-The forms are available at issuing houses, stockbroking companies and financial institutions.
Following are some steps for IPOs application forms.
-Complete the application form as indicated.
-Personal details must be the same details on the identity card or authorisation card.
-Attach the banker's order payment slip for the amount of shares applied.
-Applications can be sent via post or direct to the issuing house.

For some IPOs, applications can also be made through the Electronic Share Application (ESA) facility.
ESA allows investors to apply for shares via the ATMs of participating financial instituitions.
Following are some steps to be taken when applying for IPOs via the ESA facility.
-You must have a CDs account, savings account and an ATM card issued by a participating financial instituitions.
-Follow all instructions as required at the ATM. You must have the following details:
Personal identification Number (PIN).
CDS account number.
IPO stock code.
-Keep the ATM slip as proof of your transaction.

The calculator will be using the following rule to calculate Transaction Cost for both buy and sell trade:
1. Brokerage Rate
Standard Rate 0.70% - For transaction value at or below RM100,000
0.60% - For transaction value exceeding RM100,000 and at or below RM500,000
0.50% - For transaction value exceeding RM500,000
Special Rate Brokerage Rate specified by user in the calculator.
The effective rate that will be used for calculation will be the Brokerage Rate specified by user in the calculator which is not 0.70%
A minimum fee of RM2 for bonds/loan stocks and RM28 for normal stocks/warrants.
2. Clearing fee 0.03% of transaction value with a maximum of RM1000.00 per contract
3. Stamp duty RM1.00 for RM1,000 or fractional part of transaction value.
